Physical Exam In-Office

Fitting

Follow-Up Appointment

What to bring to your first appointment:

• Your current orthosis/prosthesis even if it has been outgrown or is broken.
• Current insurance information and a current prescription.
• For all initial appointments you must present a prescription.
• Please bring a form of identification.
• Depending on the type of visit you may need to bring shorts, shoes or other apparel for your appointment.

Selecting a Practitioner

Selecting a prosthetic provider is a big decision. Here are some things to consider about the prosthetist and the office staff during your decision making process.
• Education, training and certifications
• Personality and style
• Office hours and location
• Ask to speak to existing patients if possible
• Schedule a visit and consultation

Insurance

Our Reimbursement Team will work with you navigate your provider’s guidelines in order to help you secure coverage for your device. It’s important to note that every patient’s coverage may be different based on several factors including the nature of your disability and your provider’s specific guidelines.
